Client Consent & Liability Release Agreement The Mind Fairy, LLC By participating in any services offered by The Mind Fairy, LLC, I understand that guided meditation, energy healing, and oracle or angel card readings are intended for relaxation, personal insight, and spiritual support only. These services are not medical, psychological, legal, financial, or professional treatment or advice, and no specific outcomes are guaranteed. I understand that I am responsible for my own physical, emotional, and mental well-being at all times. I affirm that I am able to safely participate and that I may stop a session at any time. I voluntarily assume all risks associated with my participation and release and hold harmless Dana Hice DePugh and The Mind Fairy, LLC from any and all liability, injuries, decisions, or outcomes that may result from these services. I understand that guided meditation may involve sitting, standing, or gentle movement, and that energy healing is a natural, non-invasive technique for stress reduction and relaxation. I acknowledge that energy work does not diagnose, treat, or replace any form of medical or mental health care, and I agree to consult licensed professionals for any physical, psychological, or health-related concerns. I understand that oracle and angel card readings are for spiritual guidance and entertainment purposes only, and are not a substitute for medical, legal, financial, psychological, or business advice. Any actions or decisions I make based on a reading are entirely my own responsibility. I acknowledge that readings are available only to individuals age 18 or older. If I experience emotional distress at any time, I will inform the practitioner immediately. I understand that the practitioner may end a session at any time if necessary. Confidentiality will be maintained except where disclosure is required by law, and I accept full responsibility if I choose to share session content with others. I understand that if I choose to end a reading within the first 10 minutes, no payment is required. After 10 minutes, I agree that a service has been provided and full payment is due. By providing my electronic signature, typed name, or clicking “Yes, I agree,” I acknowledge that my electronic signature has the same legal effect as a handwritten signature. I confirm that I have read, understand, and voluntarily agree to this entire consent and release agreement, and I may request a copy at any time.
